Under the adult plan, additional discounts are eligible per additional family member:

• 2 people: $400/yr per person
• 3+ people: $365/yr per person

*Patients must remain on the plan throughout the duration of treatment in order to receive benefits. Payment for treatment not included in the membership plan is due at the time of service. This program is a discount plan, not an insurance plan, and is secondary to any other dental plan. It cannot be used for any treatment outside of the office, and for any treatment referred to specialists. The savings offered in the membership plan cannot be combined with any other service. Membership plans run for 12 months and becomes effective immediately once paid for.
